How to save Mairin and defeat Aunt Ethel
As you head towards Aunt Ethel's house, you will meet young people who are looking for their sister Myrina. Helping them will be difficult, but it is worth paying attention to them. Here's how to complete the mission to save Mairin:
Talk to Aunt Ethel in the Hollow in the Emerald Grove and tell her about the mind flayer tadpole in your brain. She will invite you to her home and a quest marker will appear on the map.
Find Mairin's brothersarguing with Aunt Ethel. If you support the brothers, you can return Mairin to the family, and Aunt Ethel will leave.
Thereafter go to Aunt Ethel's house. Meet Mairin and tell her about her brothers or don't say anything.
If you tell Mairin about her brothers, Aunt Ethel will grab her and try to run away. If you don't, she teleports Mairin to another location.
You can talk to Mairin about her brothers, then let her take one of your eyes or interact in a hostile manner.
To continue the quest, interact with the fireplace so that it does not catch fire and enter the door at the back.
